COMMUNITY CONNECTION PROGRAM 
Our Community Connection programming, offered in both Denver and Colorado Springs, utilizes the community as a learning environment to provide individuals with access to, and participation in, typical activities and functions of community life.  Our services provide a variety of opportunities to facilitate relationships and natural support in the community through activities such as volunteer work, community education or training, and retirement activities.

We develop programs for each client based on their interests, needs, and abilities. We recognize that many of our clients need an opportunity to meet peers and make friends. We match individuals with similar interests and provide a staff-to-client ratio of no more than one to three. This allows us to take advantage of many recreational and cultural opportunities available in the Denver and Colorado Springs area while encouraging peer relationships to develop.

SPECIALIZED HABILITATION 
Our Specialized Habilitation (SpecHab) engages individuals in a variety of functional activities with an emphasis on skill development.  SpecHab supports individuals in enhancing their well-being, creating healthy relationships, navigating challenges, and improving their overall quality of life.

Our program helps people learn or maintain a range of life skills in any of the following categories: Self-Empowerment, Safety Awareness and Self-Advocacy, Interpersonal Effectiveness and Effective Social Communication, Coping Strategies for Everyday Life Challenges, Managing Daily Tasks, and Learning New Skills.

Adult Residential

Offered in Denver

EVANS HOUSE
We focus on a person’s ability, not their disability. Located in Denver, residents of Evan’s House have extensive medical needs or a disability requiring total care. The goal of this program is to create a setting that is as normal and homelike as possible while maintaining personal dignity. Our staff members are trained to increase the quality of life for those in their care while focusing on the client’s physical, mental, emotional, and spiritual needs. We have a culture of respect and believe community interaction and personal accomplishment are important.

Services:

• Medication administration

• G-tube feeding (if needed)
• Physical, occupational, speech, an respiratory therapy
• Nursing consultation
• Individualized treatment goals
• Sensory stimulation
• 24-hour away supervision

PERSONAL CARE ALTERNATIVE PROGAM
Serving the greater Denver metro area, the PCA program helps a person live in his or her own home or host home. Staff members are available for daily, weekly, or monthly support while providing training in daily living skills, money management, meal preparation, and community and safety skills. The goal of this program is to offer support so individuals can have a quality lifestyle in a typical neighborhood setting while taking advantage of what the community has to offer.

Services:

• Teach daily living and domestic skills in a person’s own home.
• Follow-along support for money management and medical appointments.
• Provide 24-hour-a-day phone support.
• Advocate with day programs and other service providers.
• Participate in recreation and leisure skill development.
• Follow through on Individual Service Support Plan goals.

Family Caregiver

The Family Caregiver Program allows an individual to receive service with paid support from a family member. This service needs to be rendered in the individual’s own home. The family member is required to take necessary training and must be able to provide the service when needed physically. The family member will be contracted to provide the service. The goal of this program is to offer support so individuals can stay in their family home while getting support from the agency to navigate waiver services and community resources.
